Care Management Information

The Care Management Department is a cohesive group of professionals who plan and design and implement programs for children, adults, persons with a serious mental illness (SMI), substance abusers and to the community. Each area provides a special expertise in providing clinical consultation, training and technical assistance, including wellness services to provider agencies, families, enrolled members, and stakeholders throughout the Northern Arizona communities.
